Purpose & Impact:

The role of the Child Welfare Support Specialist is to deliver comprehensive case management services to LSF clients and their families, fostering meaningful relationships while embodying the agency's core values.

Key Responsibilities:
  • Accept and manage all cases assigned by the unit supervisor, ensuring timely engagement with children and their families.
  • Collaborate with families to develop tailored case plans that address their unique strengths and needs, aiming for reunification or other permanent solutions.
  • Conduct regular visits to children in their homes, maintaining a schedule that includes unannounced visits.
  • Document all case activities accurately in the designated database within 48 hours, ensuring compliance with established protocols.
  • Facilitate timely referrals to community services and maintain up-to-date knowledge of available resources.
  • Prepare and submit necessary court documentation within required timelines and actively participate in court proceedings.
  • Conduct thorough child safety assessments and develop safety plans as needed.
  • Engage in diligent searches for family members when necessary, ensuring all efforts are well-documented.
  • Conduct home studies for potential placements and provide relevant background information to prospective caregivers.
  • Utilize personal transportation to assist children as required.
  • Complete all Independent Living tasks as mandated.
  • Participate in all required meetings, training sessions, and workshops.
  • Serve as an on-call Case Manager as scheduled.
  • Collaborate effectively with various stakeholders, including Program Directors, other Case Managers, and legal staff.
  • Manage time efficiently to meet all case management requirements and deadlines.
  • Adhere to all relevant laws, policies, and court orders in case management practices.
  • Organize and prioritize tasks to meet established deadlines while maintaining a high standard of professionalism.
  • Exhibit strong communication skills, keeping supervisors informed of all relevant activities and issues.
  • Foster teamwork and support the overall goals of the organization.
  • Adhere to safety protocols in the workplace.
  • Complete all required training and maintain certifications as necessary.
  • Uphold confidentiality standards at all times.
Qualifications:

Physical Requirements:
  • Valid driver's license and insurance.
  • Ability to travel locally and attend professional development events.
  • Proficient in computer use and capable of producing written documentation.
  • Flexibility to adapt to varying work hours and responsibilities.
  • Certification in First Aid/CPR and crisis intervention.
Education/Experience:
  • Minimum of one year of relevant experience, with certification required within one year.
  • Bachelor's degree in a Human Services field, preferably in Social Work.
Skills/Abilities:
  • Exceptional written and verbal communication skills.
  • Proficient in Microsoft Office applications.
  • Ability to travel as required for job responsibilities.
  • Understanding of case management responsibilities and the importance of trauma-informed care.
  • Demonstrated sensitivity to the cultural and socioeconomic backgrounds of clients.
