Our teams travel to patients residing in various community settings, including Assisted Living, Memory Care, and Group Home facilities across multiple states.


Position Summary:
We invite you to join our team as a Senior Care Services Coordinator. In this role, you will engage with the senior demographic, managing essential activities mandated by the Department of Human Services (DHS) and the Center for Medicare/Medicaid Services (CMS) for members living in community and assisted living environments.

This position allows for remote work, with regular travel within your local area to meet the needs of your members and community.


Work Schedule:
Full-time position with day shift hours, excluding evenings, weekends, and holidays. Standard hours are 8 AM to 5 PM from Monday to Thursday and 8 AM to 3 PM on Fridays.
Key Responsibilities:
As a Senior Care Services Coordinator, you will oversee member caseloads within your designated geographic area. Your responsibilities will include:
  • Coordinating in-person visits
  • Managing the Elderly Waiver program
  • Conducting annual assessments, including Personal Care Assistance (PCA) evaluations for community members and customized living assessments for those in Assisted Living
  • Reviewing current health needs, setting goals, and creating personalized care plans
  • Connecting members with community and state resources and services
  • Completing necessary documentation
  • Collaborating with medical teams to ensure healthcare quality standards are met and utilizing management tools to achieve value-based objectives
  • Supporting members during transitions of care and working with their care teams to ensure safe discharges and follow-up plans

Qualifications:
Education/Certification/Experience:
- Must possess a Minnesota Licensed Social Worker credential (new graduates are encouraged to apply) or a current Minnesota Registered Nurse license.
- Preferred: One or more years of experience in case management or care coordination within geriatric populations, including Home Care, Nursing Home, Transitional Care Units, or Assisted Living settings.
- A valid driver's license is required.

- Strong creative problem-solving skills.

- Proven effective communication skills with strong verbal abilities.
- Excellent interpersonal and customer service skills.

- Proficiency in reading, writing, speaking, and understanding the English language.
