Presbyterian Homes & Services – Optage Hospice is in search of dedicated registered nurses who are passionate about hospice care and wish to support older adults in a compassionate Christian setting. We are looking for a registered nurse to become part of our team as a Hospice Care Coordinator, serving the North and East metro regions.

Role and Responsibilities

In this role, you will be instrumental in providing care for older adults during their final stages of life, addressing their physical, emotional, social, mental, and spiritual needs. This is a full-time position with daytime hours from Monday to Friday, along with participation in a fair on-call rotation.

Key Responsibilities

The Hospice Care Coordinator is responsible for ensuring the safe and quality delivery of hospice services to clients in various settings, including their homes, skilled nursing facilities, or assisted living environments. Our coordinators develop, implement, and assess care plans, making necessary adjustments to address all client needs. They facilitate the coordination of hospice services, ensuring continuity of care with other healthcare professionals while identifying interdisciplinary care requirements.

Essential duties include:

  • Conducting ongoing assessments of the physical, psychological, social, and spiritual needs of patients and their families
  • Providing education, guidance, and support to clients and their caregivers regarding care requirements
  • Maintaining accurate and appropriate documentation of patient and family services

Qualifications

  • A strong commitment to serving older adults with a focus on end-of-life care
  • Current registered nurse licensure in the state(s) of practice
  • A minimum of one year of nursing experience is preferred
  • Background in critical care, oncology, long-term care, or medical/surgical nursing with a solid understanding of pain and symptom management is preferred
  • Experience in hospice or home care is advantageous but not mandatory
  • Excellent interpersonal and communication skills
  • Ability to work effectively with patients and caregivers in various settings
  • Capability to collaborate as part of an interdisciplinary team
  • Strong assessment skills with experience in symptom management preferred
  • Proficient oral and written communication skills
  • Computer literacy is beneficial
  • Reliable transportation, valid driver's license, and current auto insurance
