Face to Face Health and Counseling is a non-profit organization committed to promoting economic and health equity for young individuals aged 11–24.

JOB SUMMARY:


The Youth Services Intake Coordinator is tasked with performing intake assessments at SafeZone, our day shelter for youth facing homelessness.

This role involves engaging with new youth, gathering essential information, evaluating their immediate needs, and linking them to available on-site and community resources.

Additionally, this position acts as the primary Coordinator for youth assessments in Ramsey County. The Coordinated Entry System serves as a gateway to specialized housing for youth experiencing homelessness.

This role will engage with youth who are currently residing in shelters, outdoors, in unsuitable living conditions, or escaping domestic violence to complete a Coordinated Entry Assessment.

This assessment determines youth placement on a prioritization list for housing programs based on their unique histories and needs.

This position requires a commitment of 37.5 hours per week.

RESPONSIBILITIES:
Conduct intakes with youth entering SafeZone, ensuring accurate and timely data entry into our youth database (Apricot);

Connect new youth to available services and supports at Face to Face, particularly addressing immediate needs;

Facilitate Coordinated Entry assessments with youth experiencing homelessness, both by appointment and on a walk-in basis;

Accurately input assessments into HMIS (statewide database) in a timely manner;

Assist with daily operational tasks at SafeZone as required;

Train new staff on intake procedures and Coordinated Entry assessments;

Stay informed on updates to CEYS and HMIS requirements, communicating changes to the team as necessary;

Collaborate with county-wide Coordinated Entry partners to monitor the status of youth on the priority list as needed;

Build and maintain relationships with youth-serving programs both internally and externally;

Deliver exceptional customer service during intakes and CEYS assessments, employing a harm reduction and trauma-informed approach;

Provide all services in accordance with evidence-based principles aimed at assisting youth in overcoming homelessness;

Engage with youth in a respectful and meaningful manner;

Respond to crises and de-escalate situations calmly and professionally;

Work collaboratively with other staff, fostering a team-oriented environment; and,

Perform additional duties as assigned.



QUALIFICATIONS:

A high school diploma or equivalent is required;

A Bachelor’s degree in social services or related field is preferred; extensive experience may be considered in lieu of a degree;

A minimum of 2 years of experience working with homeless or under-resourced youth;

Ability to work independently while exercising sound judgment in involving others when necessary;

Experience with positive youth development and harm reduction methodologies;

Strong interpersonal and communication skills;

High reliability and integrity;

A positive attitude and a commitment to meeting youth needs;

Effective crisis management skills and the ability to handle stressful situations well;

Knowledge of homelessness and its impact on youth;

Demonstrated ability to communicate effectively with individuals from diverse ethnic, economic, racial, and cultural backgrounds;

Strong time management skills and the ability to prioritize tasks; and,

Attention to detail and proficiency in quick and accurate data entry.
