We are currently looking for a Client Intake Coordinator to join our dedicated team at the 7th Ave Outpatient Clinic. This facility specializes in providing mental health care, including medication management and support for individuals facing addiction challenges.

Position Overview:

The Client Intake Coordinator plays a pivotal role within our interdisciplinary team. This position is responsible for managing a significant volume of client intakes, facilitating group sessions, and conducting brief service appointments.

Key Responsibilities:

  • Conduct comprehensive screenings and assessments with clients to identify their presenting issues, enabling the development of tailored service plan objectives.
  • Create and implement initial individual service plans based on client needs.
  • Prepare necessary intake documentation, progress reports, and event notifications in compliance with regulatory standards and organizational policies.
  • Ensure timely completion of all required documentation, including screenings, assessments, and service plans.
  • Coordinate care by notifying program staff of new client intakes, summarizing intake information, and recommending appropriate services.
  • Gather essential information from new clients through courteous and professional surveys or interviews.
  • Assist in data collection to facilitate the admission process in health or mental care settings.
  • Ensure all admission forms are accurately completed and securely filed.
  • Uphold client dignity and confidentiality at all times.
  • Participate in clinical staff meetings, training sessions, and other required activities.
  • Demonstrate the ability to work independently across various sites while also functioning as an integral member of a trans-disciplinary team.
  • Be available for on-call duties after hours as assigned, including carrying a company phone and logging work hours and locations.
  • May be required to drive a company vehicle to transport clients as needed.
  • Utilize personal vehicle for business purposes when necessary.
Qualifications:
  • Must be at least 21 years of age.
  • A high school diploma is required, along with a minimum of six weeks of experience in behavioral health.
  • Must maintain personal auto insurance in accordance with organizational guidelines.
  • Fingerprint clearance from the Arizona Department of Public Safety is required or must be obtained within eight weeks of hire.
