We are seeking a dedicated Intake Specialist to facilitate the initial engagement of clients entering New Freedom's Intensive Outpatient Program. This role serves as a crucial first point of contact, ensuring a seamless and welcoming experience for new members. The Intake Specialist is responsible for assessing eligibility and outlining program expectations prior to the client's meeting with their designated clinical team. A comprehensive understanding of our treatment philosophy, admission protocols, and program criteria is essential for success in this position. Knowledge of substance use disorders and co-occurring conditions is also necessary.

Key Responsibilities:

  • Initiate and manage the lobby packet paperwork and consent/admission documentation.
  • Conduct thorough intake assessments, accurately identifying and documenting relevant diagnoses.
  • Develop individualized treatment plans based on the goals established during the assessment process.
  • Clearly explain program details to clients, ensuring they understand informed consent, including:
    • State-specific requirements and program policies.
    • Verification of client involvement in collaborative treatment options.
    • Importance of adherence to treatment protocols.
    • Resources available for co-occurring conditions.
    • Clients' rights and responsibilities.
    • All available program options and related rights.
    • Grievance and appeals procedures.
    • Feedback mechanisms regarding care quality and satisfaction.
    • Overview of the organization’s services, hours of operation, and confidentiality policies.
    • Program policies regarding substance use and safety.
    • Identification of the service coordinator for each client.
    • Provide a comprehensive member handbook detailing program expectations and rules.
    • Educate clients on advance directives.
    • Clarify the purpose and process of the assessment.
    • Ensure voluntary participation in the program.
    • Adhere to Federal Confidentiality Regulations.
    • Conduct orientation for new admissions within seven working days of treatment commencement.
  • Facilitate a smooth transition to clinical and support teams upon successful completion of intake tasks.
  • Meet established Key Performance Indicators (KPIs) to maintain high standards of client care and engagement.
  • Participate in regular clinical supervision and fulfill continuing education requirements.
  • Perform additional duties as assigned.
