Position Overview:
Southwest Behavioral & Health Services, a prominent non-profit organization in Arizona, is dedicated to providing a wide range of behavioral health services tailored to the unique needs of the communities we serve. We are currently looking for an Intake Support Specialist to become a vital part of our team at the 7th Ave Outpatient Clinic, which specializes in mental health treatments and addiction support.

Key Responsibilities:
  • Conduct comprehensive screenings and assessments with clients to identify their specific needs and formulate appropriate service plan objectives.
  • Develop and implement initial individual service plans, ensuring all intake documentation, progress notes, and notifications are prepared in compliance with regulatory standards and organizational policies.
  • Ensure timely completion of all necessary documentation, including screenings, assessments, and service plans.
  • Facilitate effective coordination of care by notifying program staff of new client intakes and summarizing relevant intake information and recommended services.
  • Gather essential information from new clients through courteous and professional surveys or interviews.
  • Collect data to assist with the admission process in a healthcare or mental health setting.
  • Maintain the confidentiality and dignity of all clients.
  • Participate in clinical staff meetings, training sessions, and other required activities.
  • Work independently across multiple sites while also collaborating as part of a trans-disciplinary team.
  • Be available for on-call duties after hours as assigned, including carrying a company phone and logging work hours and locations.
  • Drive a company vehicle or personal vehicle for business purposes as needed.
Qualifications:
The ideal candidate will possess a minimum of a high school diploma and at least six weeks of experience in behavioral health. Candidates must be at least 21 years of age and maintain personal auto insurance as per organizational guidelines. Additionally, fingerprint clearance from the Arizona Department of Public Safety is required or must be obtained within eight weeks of hire.
