Job Overview

POSITION SUMMARY:

The Program Supervisor/Coordinator's primary responsibility is to guide and educate personnel within Jmkay Healthcare Services, ensuring the delivery of exceptional direct care while overseeing the program's compliance with licensing, objectives, and care standards. This role involves providing leadership and oversight to all direct care staff at various locations while also engaging in direct care activities for clients. The Supervisor will create and ensure the documentation of outcome-focused recommendations, Coordinated Service and Support Plan Addendums, and individualized behavior/therapeutic strategies for each client served, ensuring precise execution of these plans. Additionally, this position is tasked with coordinating the provision of quality client care by verifying that the care plans developed by direct support staff are comprehensive, accurate, and adhered to.

In collaboration with support staff and healthcare professionals, the Supervisor aids in the design and implementation of client services and programs that cater to the needs of both the resident community and the larger community.


KEY RESPONSIBILITIES:

A. Program Management
  • Ensures compliance with 245D licensing regulations, conducts Rule 203 license visits, maintains required documentation, and prepares for audits.
  • Guarantees that services align with each individual's Community Service Support Plan (CSSP) and CSSP Addendum (CSSP-A).
  • Assesses the effectiveness of service delivery and methodologies, measuring progress based on observable criteria to determine when desired outcomes are achieved as outlined in their CSSP/CSSP-A.
  • Maintains accurate documentation related to service provision in accordance with licensing standards and company policies.
  • Oversees financial responsibilities for supported individuals and/or programs as necessary.
  • Develops and supervises the housing budget and quality improvement initiatives to collect, monitor, and analyze client outcomes and other critical metrics.
  • Facilitates the intake process for new residents, ensuring that all required documentation is accurately prepared.
  • Organizes existing resident files, ensuring compliance with 245D documentation and Community Service Support Plans.
  • Manages grocery procurement and budgeting for each location.
  • Completes monthly financial management tasks for households and individuals.
  • Coordinates and attends routine and specialized medical appointments, documenting and communicating any changes or concerns to the support team.
  • Oversees all medication administration, including prescription and over-the-counter medications, reporting to the RN as necessary.
B. Staff Management
  • Maintains effective communication and collaboration with all stakeholders.
  • Oversees the orientation and training of new employees, ensuring all on-site elements are satisfactorily completed.
  • Develops a staffing schedule that meets the program's needs as identified in the CSSP and service agreements while adhering to budget constraints.
  • Provides supervision and leadership to Direct Support Professionals, including managing their scheduling, training, and overall performance.
  • Ensures compliance with company training guidelines and maintains accurate training records in accordance with 245D regulations.
  • Builds relationships with service providers and coordinates their services for clients receiving home care.
  • Conducts periodic audits of client records and supervises staff to ensure adherence to agency procedures and quality service delivery.
  • Responsible for staff recruitment, disciplinary actions, and terminations, as well as determining pay rates and developing employee recognition programs.
  • Facilitates staff meetings as needed and actively seeks feedback to enhance operations and client outcomes.
  • Adheres to all regulations, policies, and budgetary guidelines of Jmkay Healthcare Services.
  • Assists staff with daily responsibilities when necessary.

  • Understand and maintain confidentiality in accordance with HIPAA regulations.

Qualifications:
  • Bachelor's degree in Human Services, Behavioral Health, Social Services, or a related field with at least one year of direct experience in the mental health/social services sector, or
  • Associate's degree in Human Services, Behavioral Health, Social Services, or a related field with a minimum of three years of direct experience in the mental health/social services sector.
  • One year of supervisory experience or two years as a team lead.
  • Experience working with individuals with intellectual and developmental disabilities, mental health issues, chemical dependency, or maladaptive behaviors.
  • Excellent verbal and written communication skills.
  • Proven ability to establish and maintain professional relationships with peers and stakeholders at all organizational levels.
  • Proficiency in MS Word, Excel, and Outlook.
  • Strong organizational and communication skills.
  • Ability to pass a background check.
  • Valid driver's license with a good driving record.
  • Willingness to work flexible hours and in various locations.
