Job Overview

POSITION SUMMARY:

The Program Director/Coordinator is responsible for overseeing and training personnel within Jmkay Healthcare Services Inc., ensuring the delivery of exceptional direct care while managing the overall program's compliance with licensing, objectives, and client care standards. This role includes providing leadership and guidance to all direct care staff at various locations while also engaging in direct care activities for clients.

The Program Director will create and ensure documentation of outcome-driven recommendations, Coordinated Service and Support Plan Addendums, and individualized behavior/therapeutic plans for each client. Additionally, this position is tasked with ensuring the quality of client care by verifying that the care plans developed by direct support staff are comprehensive, accurate, and adhered to.

In collaboration with support staff and healthcare professionals, the Program Director plays a crucial role in developing and implementing client services and programs that address the needs of both the resident community and the wider community.


KEY RESPONSIBILITIES:

A. Program Management
  • Ensure compliance with 245D licensing regulations, conduct necessary audits, and maintain required documentation.
  • Guarantee that services align with each individual's Community Service Support Plan (CSSP) and its Addendum (CSSP-A).
  • Assess the effectiveness of service delivery and progress towards individual outcomes based on measurable criteria.
  • Maintain accurate documentation related to service provision in accordance with licensing standards and company policies.
  • Oversee financial responsibilities related to client support and program management.
  • Develop and monitor budgets and quality improvement processes to analyze client outcomes.
  • Facilitate the intake process for new residents, ensuring all required documentation is in place.
  • Organize existing resident files to ensure compliance with 245D and CSSP documentation.
  • Manage grocery shopping and budgeting for each location.
  • Oversee monthly financial management tasks for households and individuals.
  • Coordinate and document medical appointments, communicating any changes to the support team.
  • Administer household medications and report to the Registered Nurse.
B. Team Leadership
  • Foster effective communication and collaboration among all stakeholders.
  • Oversee the orientation and training of new employees.
  • Manage staffing schedules to meet program needs within budget constraints.
  • Provide supervision and leadership to Direct Support Professionals, including scheduling and performance oversight.
  • Ensure compliance with training requirements and maintain accurate training records.
  • Build relationships with service providers to coordinate care for clients.
  • Conduct audits of client records to ensure adherence to agency policies and state regulations.
  • Handle personnel matters, including hiring, discipline, and employee recognition programs.
  • Facilitate staff meetings and encourage feedback to enhance operations and client outcomes.
  • Adhere to all operational regulations and budgetary guidelines.
  • Assist staff with daily responsibilities as needed.

Qualifications:
  • Bachelor's degree in Human Services, Behavioral Health, Social Services, or a related field with at least one year of relevant experience, or an Associate's degree with three years of experience.
  • Minimum one year of supervisory experience or two years as a team lead.
  • Experience working with individuals with intellectual and developmental disabilities or mental health challenges.
  • Strong communication and organizational skills.
  • Proficiency in MS Word, Excel, and Outlook.
  • Ability to pass a background check and possess a valid driver's license.
  • Willingness to work flexible hours.
