Position: Shelter Plus Care Case Manager

Organization: Lutheran Social Services of Central Ohio is actively seeking a dedicated Shelter Plus Care Case Manager to join our team. This role is crucial in supporting individuals experiencing homelessness at our affiliated shelter.

Key Responsibilities:

  • Collaborate with residents to formulate actionable, individualized plans aimed at fostering self-sufficiency and stability.
  • Maintain comprehensive case files, ensuring accurate and timely documentation of all interactions, including case notes and statistics as mandated by the program.
  • Design and implement planning frameworks that align with residents' strengths, needs, and responsibilities to facilitate goal achievement.
  • Work in conjunction with Resource Specialists to assist clients in exploring income opportunities, job training, educational resources, and employment options.
  • Provide residents with information regarding available community resources.
  • Ensure meticulous data management and documentation, including monthly budget reports and records of all resident engagements.
  • Assist clients with transportation needs related to housing transitions, grocery shopping, and other essential activities.
  • Support new shelter plus care clients in their housing search efforts.
  • Ensure a clean, safe, and well-maintained living environment for residents, encouraging their participation in upkeep.
  • Conduct orientations for all new shelter plus care participants.
  • Engage in monthly meetings and maintain necessary documentation.

Qualifications:

  • A Bachelor’s degree in social work or a related field is required, or equivalent education and experience.
  • Prior experience in homelessness services, mental health, or related fields is essential.
  • Experience in the mental health sector is required.
  • A valid driver's license, reliable transportation, and compliance with insurance standards are mandatory.
