Job Description

Job Summary:

We are seeking a highly skilled Care Coordinator to join our team at Integrated Services for Behavioral Health. As a Care Coordinator, you will play a critical role in providing exceptional care to children, youth, and families with significant behavioral health needs.

Key Responsibilities:

  • Service Linkage and Care Coordination: Develop and implement individualized care plans that address the unique needs of each child, youth, and family.
  • Family Engagement: Build strong relationships with families to identify their strengths, needs, and goals, and work collaboratively to achieve them.
  • Community Resource Development: Identify and develop partnerships with community resources to provide comprehensive support to children, youth, and families.
  • Documentation and Reporting: Maintain accurate and timely documentation of care coordination activities, including progress notes, care plans, and outcome measures.
  • Collaboration and Communication: Work effectively with multidisciplinary teams, including child and family teams, to ensure seamless care coordination and communication.

Requirements:

  • Experience: A minimum of three years of experience providing services and/or support to children and families connected to behavioral health, child welfare, developmental disabilities, juvenile justice, or a related public sector human services or behavioral healthcare field.
  • Education: A high school diploma or equivalent, or an associate degree or bachelor's degree, or a master's degree or higher.
  • Skills and Qualifications: Excellent communication and interpersonal skills, with the ability to work effectively with diverse populations, including children, youth, and families with significant behavioral health needs. Strong organizational and time management skills, with the ability to prioritize multiple tasks and meet deadlines. Ability to work in a fast-paced environment and adapt to changing circumstances. Knowledge of community resources and services, including Hi-Fidelity Wraparound, and experience working with children and families with autism spectrum disorders and developmental disabilities.
