Lutheran Social Services of Central Ohio is actively seeking a dedicated Shelter Plus Care Case Manager to join our team. This role is essential in providing support to individuals experiencing homelessness through our comprehensive services.
The successful candidate will embody Our Mission: Creating a better world by serving people in need.
Key Responsibilities:
- Collaborate with residents to formulate actionable, individualized plans aimed at fostering self-sufficiency and stability.
- Maintain accurate and timely case documentation, including detailed records of meetings, referrals, and case management hours.
- Design and implement planning systems that reflect the strengths and needs of residents, outlining responsibilities and action steps.
- Work alongside Resource Specialists to facilitate access to income opportunities, job training, education, and employment.
- Provide residents with information regarding available community resources.
- Ensure meticulous data management and case notes, including monthly budget reports and documentation of all resident interactions.
- Assist clients with transportation needs related to housing transitions and grocery shopping by coordinating with local transit options.
- Support new clients in their housing search efforts.
- Ensure a clean, safe, and well-maintained living environment for residents, encouraging their participation in upkeep.
- Conduct orientation sessions for all new shelter plus care recipients.
- Engage in monthly meetings and maintain comprehensive documentation of discussions.
Qualifications:
- Bachelor's degree in social work or a related field, or equivalent education and experience.
- Prior experience in homelessness services, mental health, or related fields is essential.
- Experience in the mental health sector is required.
- A valid driver's license and reliable transportation are necessary, meeting the organization's insurance standards.
