Veteran Support Coordinator - SSVF

Full Time | Day Shift

Lutheran Social Services of Central Ohio is actively looking for a Veteran Support Coordinator - SSVF to join our dedicated team. This role is essential in providing comprehensive support to individuals experiencing homelessness, particularly veterans, ensuring they receive the necessary resources and assistance.

Key Responsibilities:

  • Collaborate with local and governmental organizations (social services, shelters, faith-based groups, correctional facilities, VA programs, legal aid, etc.) to identify very low-income veteran families facing housing instability.
  • Utilize appropriate assessment tools to evaluate the eligibility of identified families, prioritizing those in urgent need and considering alternative interventions that may better suit their circumstances.
  • Conduct intake assessments for new participants, including comprehensive needs evaluations and health screenings.
  • Guide ineligible families towards alternative community resources and VA support services.
  • Engage with clients to create and execute personalized, action-oriented goal plans focused on achieving housing stability, utilizing the Critical Time Intervention (CTI) model.
  • Establish planning frameworks that reflect client strengths, responsibilities, and actionable steps to support goal attainment.
  • Provide financial planning assistance to promote housing retention.
  • Coordinate with VA services, veteran programs, and community resources to aid clients in securing income, employment, and other essential supports.
  • Connect clients with internal resources for financial assistance, including direct payments to landlords.
  • Refer clients to partner organizations and community resources that align with their immediate and long-term objectives.
  • Work closely with all staff members to deliver integrated services and facilitate access to benefits for clients.
  • Research and compile information on available community resources, fostering relationships to ensure ongoing support for clients.
  • Maintain accurate and detailed case notes, ensuring timely submission of reports and documentation of all client interactions.
  • Participate actively in weekly case reviews, collaborating with colleagues to achieve positive outcomes for clients.
  • Support clients in maintaining ongoing relationships with community resources, family, and friends.
  • Complete Critical Time Intervention training successfully.
  • Contribute to a safe and supportive environment by adhering to the organization's Code of Ethics and Confidentiality Agreements.
  • Assist in crisis intervention and de-escalation by demonstrating professional problem-solving skills for clients and colleagues.

Qualifications:

  • A four-year degree in Social Work or a related field, or equivalent experience is required.
  • Licensed Social Worker (LSW) is strongly preferred.
  • Previous experience working with homeless populations is preferred.
  • A valid driver's license and access to reliable, insured transportation are required.
