Position Overview:
The primary responsibility is to deliver healthcare and supportive services to patients in their residence. This role involves ensuring a safe and sanitary environment, collaborating effectively with patients and their families, and communicating observations and concerns to the supervising nurse. The position operates under the guidance and oversight of a registered nurse.

Qualifications:

Educational Background:
A High School Diploma is preferred.

Completion of Required Training:
Candidates must have successfully finished one of the following:
A) A training program/competency evaluation as per Medicare regulations; B) A competency evaluation program compliant with §418.76 (c); C) A state-approved Nurse Aide training/competency evaluation program listed in good standing on the state nurse aide registry; D) A state licensure program meeting the necessary requirements.

Licensing Requirements:
A valid Driver's License is mandatory.

Experience:
A minimum of one year is preferred, with experience as a hospice aide or nursing assistant in settings such as hospitals, nursing homes, home health agencies, hospices, or long-term care facilities, or completion of a Certified Nursing Assistant or Certified Home Health Aide Program.

Skills:
Proficiency in reading and writing in English is essential, along with the ability to follow both written and verbal instructions effectively. A demonstrated commitment to the well-being of the ill and elderly is required. Candidates must successfully pass the Aide Competency Evaluation Skills Checklist that aligns with §418.76(c) and a written skills assessment.

Transportation:
Reliable transportation and valid auto liability insurance are required.

Background Check:
Candidates must consent to and successfully pass a criminal history check.

Work Environment:
The role involves working in patients' homes under various conditions, with potential exposure to blood, body fluids, and infectious diseases. Flexibility in scheduling and local travel is necessary, with some exposure to inclement weather. A Hepatitis profile is required.

Physical and Mental Demands:
The position requires prolonged standing and walking, the ability to lift up to 50 lbs., and assist patients with mobility. It involves working under some stress to meet deadlines, assess patient needs, make prompt decisions, and address psychosocial needs of patients and families. Hand-eye coordination and manual dexterity are essential, along with the ability to use durable medical equipment in a home setting.

Key Responsibilities:
- Deliver direct patient care under the direction of the RN and in accordance with the Aide Plan of Care.
- Provide necessary skills for safe patient transfers and ambulation as per agency policy.
- Accurately report changes and document relevant information and care provided to ensure continuity of care.
- Adhere to accepted infection control principles and maintain a clean, safe, and comfortable environment.
- Foster positive, supportive, and respectful communication with patients, families, and colleagues.
- Ensure respect for patient privacy and property.
- Assist patients with proper nutrition and adequate fluid intake.
- Contribute to the efficient operation of the agency and demonstrate effective time management skills.
- Perform treatments and procedures according to agency policy.
- Uphold the agency's philosophy and administrative policies to guarantee quality care.
