The Hospice Triage Nurse is a critical member of our clinical team, responsible for providing exceptional patient care and support to patients and families in need. As a Triage Nurse, you will be the first point of contact for patients and families, responding to phone calls and providing timely and compassionate care.

Key Responsibilities:

  • Respond to phone calls and provide timely and compassionate care to patients and families
  • Conduct thorough nursing assessments to identify patient needs and develop individualized care plans
  • Collaborate with the hospice team to ensure seamless care coordination and communication
  • Provide education and support to patients and families on end-of-life care and bereavement
  • Maintain accurate and up-to-date records of patient interactions and care plans

Requirements:

  • Minimum one (1) year of hospice experience required
  • Multi-state license
  • Strong organizational and self-management skills
  • Critical thinking and good judgment
  • Commitment to providing excellent customer service to patients and families
  • Valid driver's license
  • Ability to pass background checks
