- Routine Patient Visits: Perform routine patient visits, monitoring their condition and adjusting care plans as needed.
- Care Coordination: Collaborate with the hospice interdisciplinary team to ensure seamless care coordination and address patient/family needs.
- Communication: Effectively communicate with attending physicians, hospice physicians, and other healthcare professionals involved in patient care.
- Accountability and Integrity: Demonstrate accountability, flexibility, respect, and integrity in all aspects of patient care.
- Compliance: Ensure compliance with applicable legal requirements, company standards, policies, and procedures.
- Documentation: Timely submit all required paperwork and documentation.
- Current RN License: Possess a current Registered Nurse license in the state of Texas.
- Hospice Experience: Have at least one year of experience as a RN Case Manager in the hospice setting.
- Driver's License and Transportation: Hold a current Texas Driver's license and have dependable transportation.
