Position Overview:
The primary responsibility is to deliver healthcare and supportive services to patients in their residences. The position operates under the guidance and oversight of a registered nurse.

Qualifications:

Educational Background:
A High School Diploma is preferred.

Required Certifications:
Completion of one of the following:
A) Training program/competency evaluation as outlined in Medicare regulations; B) Competency evaluation program compliant with §418.76 (c); C) State-approved Nurse Aide training/competency evaluation program listed in good standing on the state nurse aide registry; D) State Licensure program fulfilling requirements of § b)(c).

Licensing:
Must possess a valid Driver's License.

Experience:
Preferred experience of at least one year as a hospice aide or nursing assistant in various healthcare settings, or completion of a Certified Nursing Assistant or Certified Home Health Aide Program.

Skills Required:
Ability to read and write in English and effectively follow both written and verbal instructions. Demonstrates a genuine interest in the care of the ill and elderly. Must successfully complete the Aide Competency Evaluation Skills Checklist as per §418.76(c) and pass a written skills assessment.

Transportation Requirements:
Reliable transportation and valid auto liability insurance are necessary.

Background Check:
Must consent to and successfully pass a criminal history background check.

Work Environment:
Work is conducted in patients' homes under various conditions, with potential exposure to blood, body fluids, and infectious diseases. Flexibility in scheduling and local travel is required, along with some exposure to inclement weather. A Hepatitis profile is necessary.

Physical and Mental Demands:
Prolonged standing and walking are essential. Ability to lift up to 50 lbs. and assist in patient mobility is required. The role involves working under some stress to meet deadlines, identify patient needs, and respond to family psychosocial requirements. Hand-eye coordination and manual dexterity are necessary for utilizing durable medical equipment in home settings.

Key Responsibilities:
- Deliver direct patient care under the direction of the RN and in accordance with the Aide Plan of Care.
- Provide necessary skills for safe patient transfers and ambulation following agency policies.
- Report changes and document relevant information to ensure continuity of care.
- Adhere to accepted infection control principles and maintain a clean, safe, and comfortable environment.
- Foster positive, respectful communication with patients, families, and colleagues.
- Ensure respect for patient privacy and property.
- Assist patients with nutrition and hydration needs.
- Contribute to the efficient operation of the agency and demonstrate effective time management skills.
- Perform treatments and procedures as per agency policies.
- Uphold the agency's philosophy and administrative policies to ensure quality care.
