Nurse Care Specialist

The Registered Nurse (RN) will be responsible for providing exceptional care and services to a group of patients, visiting patients in their homes, skilled nursing facilities, and assisted living facilities. This position is a contributing member of the interdisciplinary team, providing expertise regarding psych-social assessments, interventions, and knowledge of community resources.
Key Responsibilities- Provide high-quality patient care and services, working closely with patients and their families to understand their needs and preferences.
- Conduct thorough psych-social assessments and develop personalized care plans to meet the unique needs of each patient.
- Collaborate with the interdisciplinary team, including nurses, social workers, chaplains, volunteers, and CNA leadership, to ensure seamless care coordination.
- Work independently and on your own from day-to-day, managing visits, documentation, meetings, and other responsibilities with ease.
- Manage time effectively, prioritizing and triaging multiple concerns to ensure timely and efficient care delivery.
- Develop strong relationships with patients, families, and healthcare professionals, fostering a culture of trust and respect.
- Stay up-to-date with the latest hospice rules and regulations, ensuring compliance and adherence to industry standards.
- Current Registered Nurse (RN) licensure in good standing in the state(s) of practice.
- Emphasis placed on Spanish-speaking candidates.
- Excellent communication and interpersonal skills, with the ability to work effectively in a team environment.
- Strong time management and organizational skills, with the ability to prioritize and manage multiple tasks and responsibilities.
- Ability to learn and adapt to new electronic medical records systems.
- Reliable transportation, valid driver's license, and auto insurance, to travel across the service area.
