Work Schedule: Monday to Friday, 8 AM to 5 PM

Service Area: Various rural regions, ensuring comprehensive care for patients in need.

Key Responsibilities:

  • Providing personal care services, including bathing, grooming, and maintaining hygiene
  • Assisting patients with dressing and undressing
  • Preparing and planning nutritious meals, including grocery shopping
  • Supporting patients during meal times
  • Monitoring skin condition to prevent tissue damage
  • Communicating significant changes in patient condition to the Case Manager

Required Qualifications:

  • State certification as a nurse aide
  • Current CPR certification
  • Clear record on the state aide registry, eligible to provide patient care
  • Minimum age of 18 years, unless otherwise dictated by state law
  • Valid driver's license and insured vehicle as per state regulations
