Position Overview:
The primary responsibility is to deliver healthcare and supportive services to patients in their residences. This role involves maintaining a safe and sanitary environment, collaborating effectively with patients and their families, and communicating observations and concerns to the supervising nurse. The position operates under the guidance and oversight of a registered nurse.

Educational Background:
A High School Diploma is preferred.
Required Certifications:
Must have successfully completed one of the following:
A) Training program/competency evaluation as per Medicare regulations; B) Competency evaluation program that adheres to §418.76 (c); C) State-approved Nurse Aide training/competency evaluation program listed in good standing on the state nurse aide registry; D) State Licensure program that meets regulatory requirements.
Licensing:
Must possess a valid Driver's License.
Experience:
Preferred experience of at least one year as a hospice aide or nursing assistant in various healthcare settings, or completion of a Certified Nursing Assistant or Certified Home Health Aide Program.
Skills Required:
Proficiency in reading and writing in English, with the ability to follow both written and verbal instructions effectively. A demonstrated commitment to the welfare of the ill and elderly is essential. Must successfully complete the Aide Competency Evaluation Skills Checklist that meets §418.76(c) and a written skills assessment.
Transportation Requirements:
Reliable transportation and valid auto liability insurance are mandatory.
Background Check:
Agreement to undergo and pass a criminal history check is required.
Work Environment:
Work takes place in patients' homes under various conditions, with potential exposure to blood, body fluids, and infectious diseases. Flexibility in scheduling and local travel is necessary, with some exposure to inclement weather. A Hepatitis profile is required.
Physical and Mental Demands:
Prolonged standing and walking are necessary. The ability to lift up to 50 lbs. and assist in moving patients is required. The role involves working under some stress to meet deadlines, identify patient needs, and respond to patient/family psychosocial requirements. Manual dexterity and hand-eye coordination are essential for utilizing durable medical equipment in the home.
Key Responsibilities:
- Deliver direct patient care under the direction of the RN and in accordance with the Aide Plan of Care.
- Provide necessary skills for safe patient transfers and ambulation as per agency policy.
- Accurately report changes and document relevant information to ensure continuity of care.
- Adhere to accepted infection control principles and maintain a clean, safe, and comfortable environment.
- Foster positive, respectful communication with patients, families, and colleagues.
- Uphold the dignity and privacy of patients and their property.
- Assist patients with proper nutrition and hydration.
- Contribute to the efficient operation of the agency while demonstrating effective time management skills.
- Perform treatments and procedures in accordance with agency policy.
- Promote the agency's philosophy and policies to ensure high-quality care.
