Overview

Health Connect America is a leading provider of mental and behavioral health services, dedicated to making a positive impact in the lives of individuals and families. As a Peer Support Specialist, you will play a vital role in our team, utilizing your personal recovery experience to support and empower others.

Responsibilities
  • Provide education and support to families and communities
  • Use personal recovery experience as a tool for growth and development
  • Navigate individuals to community resources, including housing, social services, education, and employment assistance
  • Lead recovery education and activities
  • Develop and implement Wellness Recovery Action Plans (WRAP) to promote personal growth and relapse prevention
  • Facilitate peer support groups and provide opportunities for socialization in the community
  • Provide outreach to individuals who have frequent inpatient hospitalizations or have failed to engage with the behavioral health system
  • Assist individuals in developing and implementing person-centered individual service plans
  • Support individuals through the intake process and educate them on changes in healthcare reform
  • Help individuals understand their benefits and navigate the SOAR process for SSI/SSDI benefits
  • Follow up with individuals after discharge and provide ongoing support
Qualifications
  • Must be at least 18 years old
  • Must have a mental health condition and/or substance use disorder, be in recovery, and be open-minded
  • Must be willing to share personal experiences with mental illness/substance use publicly
  • Must have a High School diploma or GED
  • Must have a valid driver's license in the state of employment with a safe driving record
  • Must have successfully completed state-specific Peer Specialist training and exam
