Job Overview

V-Tech Solutions, Inc. is seeking a dedicated Community Outreach Representative to enhance our client services. The ideal candidate will be meticulous and enthusiastic about delivering exceptional care and support to individuals in need. This role involves providing field services, including:

  • Conducting home visits to begin the enrollment process for newly eligible individuals.
  • Tracking existing members as part of a monitoring initiative.
  • Educating both prospective and current members and their families about available services and benefits.
  • Supporting the Family Support Network and Youth Athletic Program, while representing the organization at health fairs and community events.

While the primary focus will be on the Latino community, the representative will engage with the broader community as well. This position operates under the guidance of the Director of the Department of Family and Community Development, allowing for some independent decision-making.

Key Responsibilities
  • Assist various business lines in retaining qualified members through a strategic daily schedule that includes outreach via phone and community visits.
  • Maintain accurate records of all member interactions, including outreach and home visits.
  • Ensure timely completion and submission of all necessary assessments and documentation.
  • Work towards achieving daily and monthly membership goals.
  • Contribute to the development of marketing strategies and community partnerships to raise awareness of services.
  • Conduct home visits for newly eligible members to gather essential health and needs information.
  • Support the implementation of initiatives aimed at increasing member health visits.
  • Represent the organization at local health fairs and community events.
  • Assist in organizing and facilitating outreach program activities.
  • Prepare and submit reports on outreach activities and assessments.
Qualifications

Education: High School Diploma or GED

Experience: A minimum of 3 years in community relations or customer service within a health-related or managed care organization is preferred.

Skills:

  • Exceptional customer relations skills.
  • Strong oral and written communication abilities.
  • Capability to effectively present information to various audiences.
  • Ability to work independently and collaboratively within a team.
  • Possession of a valid driver's license and reliable transportation.
