Key Responsibilities:
  • Relationship Management: Build and sustain connections with management personnel at designated shelters; identify a primary contact to refer residents as potential clients for Day Treatment.
  • Direct Engagement: Collaborate with shelters and Community Support Workers (CSWs) to pinpoint suitable individuals in need of Day Treatment Services.
  • Resident Meetings: Conduct meetings with shelter residents referred by shelter management as necessary.
  • Transportation Coordination: Develop a schedule to transport identified shelter residents to Day Treatment at PABC for engagement with intake specialists.
  • Reporting: Submit quarterly reports detailing the number of referrals identified and engaged.
  • Supervision: Report directly to the Director of Day Treatment Services.
  • Outreach Expansion: In addition to the identified shelters, the specialist will seek out additional locations, agencies, and wrap-around service providers; establish partnerships and collaboration opportunities to ensure a steady flow of referrals for Day Treatment programs.
  • Supervision Participation: Engage in weekly supervision sessions.
  • Training Attendance: Participate in all mandatory company training sessions.
Qualifications:
  • High School Diploma or equivalent is required.
  • Some college education is preferred.
  • Exceptional verbal and written communication skills.
  • Able to thrive in a diverse environment.
  • Basic proficiency in computer skills, including Microsoft Office Suite.
