POSITION SUMMARY

The Case Coordinator for Veteran Support Services plays a pivotal role in assisting individuals and families within the veteran community who are facing housing instability. This position is integral to the Back Home SSVF (Supportive Services for Veteran Families) initiative, which focuses on providing rapid rehousing solutions through a structured 90-day model aimed at securing stable housing for veterans at risk of homelessness.

ROLE AND RESPONSIBILITIES

The Case Coordinator will:

1. Facilitate the delivery of essential support services to veteran households, ensuring alignment with established service modalities.
2. Conduct comprehensive needs assessments and eligibility evaluations for program participation.
3. Engage in community outreach to promote program awareness and accessibility.
4. Provide case management services that connect veterans to necessary public benefits, VA resources, health referrals, and financial assistance.
5. Maintain a commitment to professional development by staying informed about best practices and standards in the field.

QUALIFICATIONS

To be considered for this role, candidates must possess:

- A minimum of four years of relevant experience in human services or a related field.
- A valid driver's license and state-required automobile insurance.
- Access to a personal vehicle for work-related travel, with mileage reimbursement available.
- Completion of agency and program credentialing within the first 12 weeks of employment.

WORK SCHEDULE AND COMPENSATION

This is a full-time position with a competitive salary range starting at $24.00 per hour, commensurate with experience. Additional benefits include:
