Rate of Pay :
$26.46/hour

Benefits :
Standard; Full-Time, Non-Exempt employees are eligible for but not limited to the following:

Function:

Assist families residing in temporary shelters and facilitate their transition to permanent housing. Core responsibilities will encompass housing search assistance, navigation, and ongoing case management services for participating families.

This includes conducting assessments, identifying and implementing suitable financial interventions, making referrals to community resources (such as clothing, food, and other emergency assistance), and organizing regular family case management meetings, including home visits as necessary.

The Case Manager will collaborate with various family shelters and supportive housing agencies within the community on behalf of participating families.

This role will also involve coordinating and leading participant orientation sessions. The position requires close collaboration with all members of the Housing Now team.

A proven track record of delivering professional, effective programming for participants

Ability to complete timely and thorough documentation (including assessments)

Utilize professional judgment to determine appropriate interventions.

Strong customer service skills

Competent in working independently within defined parameters in a team setting.

Bi-lingual (preferred)

Successfully pass a background check

Possess a valid driver's license and maintain a clean driving record.

Complete internal training within the first three months of employment

Understanding of HIPAA compliance

Qualification and Education Requirements:
A Bachelor's degree in human services, social work, or a related field; MSW preferred.

Experience in the housing market with a focus on homelessness prevention preferred.

Two years of experience providing housing services to vulnerable populations preferred. An equivalent combination of education and experience may be considered.

Case management experience preferred.

Must hold a valid driver's license and insurance in good standing

Knowledge, Skills and Abilities Required:


Excellent written and verbal communication skills, ability to deliver public presentations, manage databases, administer complex systems, and adhere to program policies.

Experience with federal, state, and local grants preferred. Ability to discern appropriate approaches and solutions in complex cases.

Experience in Rapid Rehousing, Housing First, Trauma-Informed Care, Wrap-Around Services, Harm Reduction, Strength-Based Case Management, Critical Time Intervention, and Crisis Intervention preferred.


Responsibilities:
Establish and maintain communication with program participants.

Maintain regular communication with the Program Director and fellow Case Managers, including participation in collaborative working group meetings.

Act as an advocate to assist participants in achieving housing stability.

Collect all necessary documentation for program intake in accordance with HIPAA guidelines.


Strength-based Case Management focused on maintaining housing stability utilizing Trauma-Informed Care, Harm Reduction, Motivational Interviewing, and wrap-around services.

Extensive record-keeping including case notes, data entry, and documentation in required systems.

Maintain comprehensive case management files and statistics as required by funding sources.

Uphold confidentiality and professional boundaries with all participants.

Collaborate closely with volunteers, interns, staff, and external agencies to facilitate program activities.


Participate in all Housing Now events.

Ability to lift up to 50 pounds

Other duties as assigned.

Qualification and Education Requirements:
High School diploma required. Bachelor's degree in Social Services, Psychology, Social Work, Public Health, or a related field required. Master's degree in social work (MSW) preferred. Two years of work experience with homeless populations, preferably with families in crisis. Experience with case management required.
