Role Overview

The Patient Flow Coordinator RN plays a crucial role in managing patient capacity and ensuring seamless patient placement. This position is essential for maintaining an efficient flow of care, addressing barriers to patient movement, and facilitating communication among various stakeholders.

Key Responsibilities

  • Utilizes clinical expertise to manage the admission and transfer processes for both elective and emergency patients, ensuring alignment with established clinical standards.
  • Possesses in-depth knowledge of medical conditions and relevant epidemiological factors that influence patient placement decisions.
  • Facilitates the daily Capacity Management strategy, ensuring all relevant staff and departments are informed and aligned with the day's objectives.
  • Communicates the Capacity Management plan effectively to evening staff and supervisors to ensure continuity of care.
  • Conducts assessments of patient conditions, collaborating with case management and nursing staff to ensure optimal placement for necessary services.
  • Maintains accurate records of bed availability and patient flow, ensuring transparency and accountability.
  • Collaborates with facilities management and environmental services to optimize room readiness.
  • Oversees daily census reports, ensuring accurate tracking of patient occupancy.
  • Engages in regular rounds on nursing units to identify discharge barriers and foster relationships with staff, enhancing the understanding of the Capacity RN's role.
  • Acts as a knowledgeable resource for patient placement staff regarding unit capabilities and equipment requirements.
  • Advises leadership on unit capacity adjustments and manages patient transfers during high-demand periods.
  • Regularly updates management on potential capacity challenges, including operating room and procedural delays.

Qualifications

  • Current NJ State Professional Registered Nurse License.
  • Bachelor's degree in Nursing from an accredited institution, with a minimum of three years of nursing experience.
  • Strong understanding of nursing principles, practices, and relevant regulations.
  • Excellent verbal and written communication skills.
  • Proven ability to maintain effective working relationships across various departments.
  • Master's degree in nursing or a related field is preferred.
  • Prior experience in capacity management is advantageous.
