About the Role

Bergen Volunteer Medical Initiative (BVMI) is seeking a highly motivated and compassionate individual to fill the position of Korean Patient Care Coordinator. This role is a vital part of our team, responsible for ensuring that our Korean patients receive high-quality, culturally responsive care.

Key Responsibilities
  • Primary Point of Contact: Serve as the primary point of contact for Korean patients, providing them with support and guidance throughout their healthcare journey.
  • Care Coordination: Liaise between BVMI clinical staff and Korean patients to ensure effective communication and care coordination.
  • Appointment Management: Coordinate and manage Korean patient appointments at the BVMI Healthcare Center.
  • Referral and Appointment Coordination: Coordinate and manage referrals and appointments at hospitals and outside specialists for Korean patients.
  • Interpretation and Translation: Provide interpretation and translation for Korean patients as needed.
  • Eligibility Interviews: Conduct eligibility interviews for Korean patients as needed.
Case Management
  • Design and Implement Case Management System: Design and implement a case management system to address social determinants of health among BVMI patients.
  • Screen for Social Determinants of Health: Meet patients in the primary health care setting to screen for social determinants of health.
  • System Navigation: Act as the system navigator to engage patients in services across the healthcare domains.
  • Referrals and Linkages: Provide referrals and linkages to address whole health and family needs.
Patient Care Coordination
  • Self-Management Goals: Work as part of the BVMI care team to assist patients in achieving self-management goals by facilitating the completion and follow-up on pre-visit planning, action plans, and care plans.
  • Collaboration Across Healthcare Domains: Facilitate collaboration across primary, specialist, and behavioral health care.
  • Telephonic and In-Person Visits: Conduct telephonic and in-person visits to ensure that the identified social determinants of health needs are addressed.
  • Electronic Medical Record: Exhibit knowledge and skill in all functions of the electronic medical record pertinent to successful job performance.
  • Documentation and Reporting: Maintain documentation in the Electronic Medical Record and other documentation and reporting as assigned.
Additional Responsibilities
  • Staff Meetings and Case Review Meetings: Participate in staff meetings and case review meetings.
  • Training and Development: Attend meetings and training as assigned.
  • Maintaining BVMI's Culture of Caring: The Korean Patient Care Coordinator will be instrumental in maintaining BVMI's culture of caring and ensuring that our Korean patients feel welcomed and well-served.
Requirements
  • Experience: 1-2 years of experience in a clinic or hospital setting (knowledge of the U.S. healthcare system and health challenges of Korean American immigrants is a plus).
  • Language Skills: Fluency in English and Korean.
  • Education: Bachelor's degree required; relevant experience may be substituted.
