Care Coordinator, Utilization Management RN

Hackensack Meridian Health is seeking a skilled Care Coordinator, Utilization Management RN to join our team. As a key member of our healthcare team, you will be responsible for coordinating, communicating, and facilitating the clinical progression of patient treatment.
Key Responsibilities- Conduct thorough reviews of patient medical records to determine the necessity of hospital stays and identify opportunities for cost savings.
- Collaborate with physicians, nurses, and other healthcare professionals to develop and implement effective care plans that meet the needs of patients and align with evidence-based practice standards.
- Communicate with payers and other stakeholders to ensure that patient care meets regulatory and contractual requirements.
- Participate in multidisciplinary rounds and contribute to the development of strategies to reduce length of stay and improve patient outcomes.
- Provide education and support to patients and families to ensure a smooth transition to post-acute care.
- Bachelor's degree in Nursing (BSN) or currently enrolled in a BSN program.
- Current RN licensure in the state of New Jersey.
- Excellent communication and interpersonal skills, with the ability to work effectively with patients, families, and healthcare teams.
- Strong analytical and problem-solving skills, with the ability to interpret complex data and make informed decisions.
- Proficiency in electronic medical records and other healthcare software systems.
