As an Oncology Nurse Navigator, you will play a pivotal role in guiding patients through their healthcare journey, starting from the pre-diagnosis phase and continuing through their treatment. Working closely with a multidisciplinary team, including primary care providers, surgeons, and oncologists, you will facilitate timely access to care and support for oncology patients. Your responsibilities will include making necessary referrals for services such as financial counseling and nutritional support.

Your daily responsibilities may encompass:

  • Coordinating and overseeing outpatient care for oncology patients, identifying new patients during their pre-diagnosis journey, and providing navigation services from diagnosis through survivorship or end-of-life care.
  • Building and nurturing professional relationships with medical oncologists, radiation oncologists, surgeons, and other healthcare professionals involved in patient care.
  • Establishing effective communication with physicians and their staff to outline navigation programs and available resources, collaborating with the oncology team to serve as a resource for patients and their families.
  • Maintaining a comprehensive communication system to ensure timely information sharing among physicians, nursing staff, and the interdisciplinary team.
  • Facilitating access to necessary services, tracking patient flow through diagnostic and treatment services, and documenting assessments and follow-ups in medical records.
  • Conducting ongoing assessments of patient needs, including educational, financial, psychosocial, nutritional, and clinical aspects, and connecting patients with available resources.
  • Participating in the Tumor Board as required.
  • For eligible patients, gathering information for the Commission on Cancer and formulating a survivorship care plan at the conclusion of treatment, discussing this plan with patients and families.
  • Engaging in performance improvement initiatives and collaborating with medical directors to enhance patient outcomes and satisfaction.
  • Participating in departmental committees and representing patient navigation services, providing detailed reports on the patients served.
  • Promoting the Cancer Program's services within the community and collaborating with marketing for outreach materials.
  • Completing other assigned duties and projects as necessary.
  • Adhering to organizational competencies and standards of behavior.

Qualifications include:

  • Bachelor of Science in Nursing (BSN) required for team members hired after March 2024.
  • A minimum of two years of nursing experience.
  • Current NJ State Professional Registered Nurse License.
  • Strong written and verbal communication skills.
  • Proficient in computer skills, including Microsoft Office and/or Google Suite.
  • Oncology experience is preferred but not mandatory.
  • Oncology Certified Nurse designation is preferred.
