Under the guidance of local leadership, the Oncology Patient Navigator plays a pivotal role in guiding patients through their healthcare journey, beginning with pre-diagnosis and continuing through the entire treatment process. Working closely with a multidisciplinary team, which may include primary care providers, surgeons, gynecologists, gastroenterologists, oncologists, and other healthcare professionals, the navigator ensures that oncology patients receive timely access to care and necessary resources.

A typical day for an Oncology Patient Navigator may involve:

  • Facilitating and coordinating outpatient services for oncology patients, identifying new patients during their pre-diagnosis phase, and providing navigation support throughout their treatment journey.
  • Establishing and nurturing professional relationships with medical oncologists, radiation oncologists, surgeons, and other healthcare providers involved in patient care.
  • Communicating effectively with physicians and their staff to outline navigation programs and available resources, acting as a liaison for patients and their families from initial consultation through treatment.
  • Maintaining a robust communication system to ensure timely and comprehensive information exchange among physicians, nursing staff, and the interdisciplinary team.
  • Tracking patient flow through diagnostic and treatment services, documenting assessments and follow-up notes in the patients' medical records as needed.
  • Conducting ongoing assessments of patient needs, including educational, financial, psychosocial, nutritional, and clinical aspects, and making referrals to appropriate resources within the healthcare system and community.
  • Collaborating with the research department as necessary.
  • Preparing for and participating in Tumor Board meetings as required.
  • For eligible patients, compiling necessary information to create a survivorship care plan at the conclusion of treatment, and discussing this plan with patients, families, and relevant providers.
  • Engaging in performance improvement initiatives and developing guidelines in partnership with Medical Directors and physician leaders to enhance patient outcomes and satisfaction.
  • Participating in department-specific committees and representing patient navigation services at meetings, providing detailed reports on the patients served.
  • Promoting the Cancer Program's services within the community and collaborating with marketing teams to create informational materials.
  • Performing additional duties and projects as assigned, while adhering to organizational competencies and standards of behavior.

Qualifications include:

  • Bachelor of Science in Nursing (BSN) required for new hires.
  • A minimum of two years of nursing experience.
  • Active NJ State Professional Registered Nurse License.
  • Strong written and verbal communication skills.
  • Proficient in computer applications, including Microsoft Office and Google Suite.
  • Oncology experience is preferred but not mandatory.
  • Oncology Certified Nurse designation is a plus.
