Job Overview

The Patient Advocate Coordinator plays a crucial role in delivering tailored, high-quality support to patients, ensuring seamless access to all clinical services within the institution and fostering a positive experience for every patient. This position acts as a dedicated navigator for patients and clients, facilitating timely access to services while providing essential information and resources related to the program(s).

Key Responsibilities:

+ Acts as a primary liaison between specialty programs, patients, clients, and service providers.

+ Manages the patient care journey, ensuring that individuals receive necessary documentation, physician directives, and follow-up appointments.

+ Communicates care plan details to patients, ensuring prompt scheduling of clinical appointments and coordinating all necessary services to deliver exceptional, patient-focused care.

+ Collaborates with international patient services to address the needs of global patients.

+ Works with health plans, clients, and patients to confirm appropriate coverage and authorization for services rendered.

+ Partners with clinical teams to enhance the overall patient experience.

+ Demonstrates a solid understanding of operational workflows and provides support as needed.

Qualifications

Education:

A high school diploma or GED is required; a Bachelor's degree is preferred.

Experience:

Preferred: Two (2) years in a concierge or liaison capacity; Three (3) years in a healthcare environment.
