Job Summary

We are seeking a highly skilled Clinical Care Coordinator to join our team at Serve the People. As a key member of our healthcare team, you will play a vital role in ensuring seamless patient care and coordination.

Key Responsibilities
  • Pre-Visit Preparation
    • Review patient charts and medical history to identify potential care gaps and develop personalized care plans.
    • Communicate with primary care providers, referral providers, and patients to ensure timely and effective care coordination.
  • Patient Appointment Facilitation
    • Coordinate patient appointments, including lab tests, diagnostic procedures, and specialty physician visits.
    • Ensure timely follow-up with patients and providers to confirm appointment details and resolve any issues.
  • Care Plan Development and Implementation
    • Work with patients and healthcare providers to develop and implement comprehensive care plans.
    • Monitor patient progress and make adjustments to care plans as needed.
  • Communication and Collaboration
    • Communicate effectively with patients, families, and healthcare providers to ensure seamless care coordination.
    • Collaborate with multidisciplinary teams to ensure timely and effective care delivery.
  • Administrative and Record-Keeping Tasks
    • Maintain accurate and up-to-date patient records and charts.
    • Perform administrative tasks, such as data entry and reporting, to support care coordination efforts.
Requirements
  • Education and Experience
    • High school diploma or equivalent required; associate's or bachelor's degree in a related field preferred.
    • Minimum 1-2 years of experience in a healthcare setting, preferably in a care coordination or patient advocacy role.
  • Skills and Abilities
    • Excellent communication and interpersonal skills.
    • Ability to work effectively in a fast-paced environment and prioritize multiple tasks and responsibilities.
    • Strong analytical and problem-solving skills, with the ability to think critically and make sound judgments.
    • Proficiency in electronic health records (EHRs) and other healthcare software systems.
