Job Summary:

The primary role of the Medical Social Worker II is to provide comprehensive social work care to patients and their caregivers, addressing the social and emotional aspects of their illness. As a key member of the healthcare team, this position delivers age-appropriate social work services in accordance with agency policy, state and federal regulations, and Kaiser Permanente's commitment to quality care.

Key Responsibilities:
  • Assessment and Intervention: Conduct psychodynamic interventions, crisis intervention, grief/bereavement counseling, problem-solving, stress reduction, and develop healthy coping strategies in individual, family, or group settings.
  • Treatment Planning: Develop and implement individual Plans of Treatment to assist patients and families in coping with and/or restoring social, emotional, financial, and environmental factors affected by illness.
  • Assessment and Planning: Complete psychosocial assessments, partner with patients to identify needs, and develop and implement individual treatment plans based on mutually agreed-upon goals.
  • Care Coordination: Discuss options for care proactively, including Kaiser resources and external community/government resources, to assist patients and families in developing short- and long-term care plans as appropriate.
  • Interdisciplinary Collaboration: Team with other disciplines in assessing, planning, and providing services for patients utilizing biopsychosocial information.
  • Patient Advocacy: Assist patients in advocating for themselves to receive appropriate services within Kaiser and in the community.
  • Discharge Planning: Assist patients and families with care planning and discharge plans.
  • Documentation and Data Management: Maintain documentation, records, and data collections, ensuring completion of required documents in a timely manner.
  • Skilled Nursing Facility Placement: Function as part of the Skilled Nursing Facility Team to ensure appropriate and timely placement of Kaiser members in nursing facilities.
  • Liaison and Communication: Liaise between patients and Kaiser, maintaining a positive relationship and providing continuity of care.
  • Level of Care Determination: Identify appropriate levels of care and facilities for referred patients, where applicable.
  • Placement and Coordination: Obtain placements, where applicable, and collaborate with internal and external resources in Kaiser and the community to meet mutually agreed-upon goals and objectives.
  • Information and Referral: Provide information and referral to community resources as requested.
  • Communication and Coordination: Coordinate the exchange of information between Kaiser, families, members, and skilled nursing facilities.
  • Benefits and Coordination: Determine the application of Kaiser, Medicare, and Medi-Cal benefits to specific patient situations.
  • Utilization Management and Quality Assurance: Participate in Utilization Management/Quality Assurance activities.
  • Communication and Coordination: Assist in coordinating communication between regional offices, clinics, hospitals, and field staff, triaging phone calls from members/families.
  • Referral and Clinical Information: Work with referral sources to clarify and complete required clinical and psychosocial information.
Requirements:
  • Experience: Step I: 0-2 years social work experience; Step II: 2-4 years social work experience within the last five years; Step III: 4 or more years social work experience within the last ten years.
  • Education: Masters in Social Work accredited by the Council of Social Work Education.
  • Additional Requirements: Demonstrated ability to work on a multidisciplinary team; solid psychosocial assessment skills; knowledge of chronic and acute disease and its impact on patient and family functioning; excellent oral/telephone communication skills and written documentation; computer literacy and experience with automated clinical information systems; ability to handle a demanding workload involving multiple tasks; ability to function independently as a collaborative, supportive team member; ability to master detailed and complex information regarding benefits and coordination of care; willingness to work in a Labor Management Partnership environment.
  • Preferred Qualifications: At least one year post-MSW experience in a healthcare setting; LCSW preferred.
