Position Overview

Role Summary:
The Community Support Specialist plays a crucial role in the planning, coordination, and management of therapeutic and rehabilitative services for individuals enrolled in Community Support Services. This position is accountable for the initial and ongoing development of the client's Person-Centered Plan (PCP), ensuring that services are implemented and coordinated effectively in alignment with the PCP. Continuous assessment of client needs, adjustments to the PCP as necessary, evaluation of intervention effectiveness, and measurement of client outcomes are essential responsibilities. The Specialist will also provide case management, supportive counseling, and skill-building interventions. Availability for evening and weekend hours is required for all team members.

Qualifications:
  • Must hold a Master's degree in a human services field and possess at least one year of full-time, post-graduate experience in mental health, developmental disabilities, or substance abuse with the relevant population; or
  • Must hold a Bachelor's degree in a human services field and have two years of full-time experience in mental health, developmental disabilities, or substance abuse with the relevant population.
