As a dedicated and friendly professional, you will play a crucial role in positively impacting the lives of individuals with intellectual and developmental disabilities, as well as those facing mental health and substance use challenges.
Role Responsibilities:
- Conduct initial outreach and engagement with members within designated regions and populations.
- Implement ongoing outreach strategies to connect with unengaged members, monitoring admission, discharge, and transfer notifications.
- Facilitate general outreach, member engagement, and follow-up, coordinating necessary services and appointments.
- Participate in health promotion initiatives and share relevant information with care managers and team members regarding members' situations.
- Provide referrals and assist in accessing community-based resources and social support services.
- Engage in case conferences, supporting care managers in identifying and addressing unmet health-related resource needs.
- Adhere to established workflows, policies, and documentation standards that foster whole person care.
- Maintain a professional demeanor and uphold the organization’s standards in all interactions.
- Education: High School Diploma or equivalent; candidates must be at least 18 years old.
- Experience: Candidates must meet at least one of the following criteria:
- Personal experience with an intellectual or developmental disability or traumatic brain injury, demonstrating knowledge of the North Carolina Medicaid system.
- Experience as a Certified Peer Support Specialist with a behavioral health condition.
- Experience as a parent or guardian of an individual with an intellectual or developmental disability or behavioral health condition, with at least two years of direct care experience navigating the Medicaid system.
- Two years of paid experience in outreach, engagement, and support roles, including direct work with Tailored Care Management populations.
- Criminal background check.
- Verification against registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities).
- Drug screening.
- Education verification and credentialing as required for the position.
- Proof of employment history or references as needed.
- For positions requiring driving, proof of driver's license, insurance, and vehicle availability.
