Position Overview:

Job Type: Regular

Compensation: $20.00 - $25.00 per hour

Sign-On Bonus: $2,500

Key Responsibilities:

  • Deliver integrated care management services across various settings, including community locations and client homes.
  • Conduct thorough assessments and develop tailored care plans in collaboration with clients and interdisciplinary teams.
  • Monitor the implementation of care plans and ensure regular updates to reflect any changes in client circumstances.
  • Foster a culture of person-centered care that emphasizes individual strengths and preferences.
  • Engage in continuous professional development to stay informed about best practices in care management.
  • Collaborate with community organizations and internal departments to enhance service delivery.
  • Maintain compliance with organizational policies and relevant regulations.

Qualifications:

To qualify as a Qualified Professional (QP), candidates must meet at least one of the following educational and experience criteria:

  • A master's degree in a human services field with a minimum of one year of relevant experience.
  • A bachelor's degree in a human services field with at least two years of relevant experience.
  • A bachelor's degree in a non-human services field with a minimum of four years of relevant experience.

Additional Requirements:

  • Valid driver's license and reliable transportation.
  • Clinical license is preferred.
  • Experience in care coordination or case management is essential.
