We are seeking a dedicated Integrated Care Coordinator to join our team at RHA Health Services. This role is essential in providing comprehensive care management for individuals with intellectual and developmental disabilities, as well as those facing mental health and substance use challenges.
Job Type: Regular
Compensation: $20.00 - $25.00 per hour
Sign-On Bonus: $2,500
Key Responsibilities:
- Deliver integrated care management services across various settings, including community locations and client homes.
- Conduct thorough assessments and develop tailored care plans in collaboration with clients and interdisciplinary teams.
- Monitor the implementation of care plans and ensure regular updates to reflect any changes in client circumstances.
- Foster a culture of person-centered care that emphasizes individual strengths and preferences.
- Engage in continuous professional development to stay informed about best practices in care management.
- Collaborate with community organizations and internal departments to enhance service delivery.
- Maintain compliance with organizational policies and relevant regulations.
Qualifications:
To qualify as a Qualified Professional (QP), candidates must meet at least one of the following educational and experience criteria:
- A master's degree in a human services field with a minimum of one year of relevant experience.
- A bachelor's degree in a human services field with at least two years of relevant experience.
- A bachelor's degree in a non-human services field with a minimum of four years of relevant experience.
Additional Requirements:
- Valid driver's license and reliable transportation.
- Clinical license is preferred.
- Experience in care coordination or case management is essential.
