Position Overview:
We are seeking an Outreach Specialist / Care Manager Extender to enhance our community engagement efforts.

Key Responsibilities:
  • Conduct outreach initiatives and initial engagement with members in designated regions and populations.
  • Maintain ongoing outreach efforts to connect with unengaged members and monitor admission, discharge, and transfer notifications.
  • Facilitate member engagement through follow-ups and coordination of services, including appointment reminders and transportation arrangements.
  • Participate in health promotion activities and share vital information with care managers and integrated care teams.
  • Assist in tracking referrals and provide support in accessing community resources and social services.
  • Engage in case conferences and collaborate with care managers to address unmet health-related needs.
  • Implement established workflows, policies, and documentation standards to promote holistic care.
  • Uphold a professional image and adhere to company policies to ensure accountability in service delivery.
Qualifications:
  • Education: High School Diploma or equivalent; must be at least 18 years old.
  • Experience: Candidates must meet at least one of the following criteria:
    • Personal experience with an intellectual or developmental disability or traumatic brain injury, demonstrating knowledge of the North Carolina Medicaid system.
    • Experience as a Certified Peer Support Specialist with a behavioral health condition.
    • Experience as a parent or guardian of an individual with an intellectual or developmental disability or behavioral health condition, with at least two years of caregiving experience navigating the Medicaid system.
    • Two years of paid experience in outreach, engagement, and service coordination, with at least one year working directly with Tailored Care Management eligible populations.
Pre-employment Requirements:
  • Completion of a criminal background check.
  • Verification against registries (OIG exclusions database, Child Abuse Registry, and Offenders Against Individuals with Developmental Disabilities).
  • Drug testing.
  • Verification of education and credentials as required for the position.
  • Proof of employment history or references, if necessary.
  • For positions requiring driving, proof of a valid driver's license, insurance, and vehicle, if applicable.
