Description and Responsibilities The Assertive Community Treatment (ACT) Team is a multidisciplinary team that provides an alternative to traditional outpatient treatment to any person who needs it within the community. The ACT team provides 24/7 care and continuity of care (open-ended, lifelong) by mimicking hospital-level wraparound support to help individuals maintain in the community rather than bouncing back and forth from the hospital. We bring the support to our individuals by meeting individuals where they’re at, in settings and spaces where they feel safe and comfortable. Our unique contract with the Columbia Center for Practice Innovations provides the ACT program robust, job-specific training to each ACT team member. Employment Specialists collaborate with the individuals enrolled in services and their collaterals to create and implement employment and/or educational treatment goals such as developing skills in resume writing, interview preparation, completing job applications, and registering for school. They work collaboratively with the team to ensure shared responsibilities of overseeing the treatment for each individual. The Employment Specialist also exhibit Open Sky’s commitment to racial equity, justice, diversity, and inclusion in the work they perform. Key Responsibilities Include: Help individuals find stable, integrated employment opportunities based on their interests, skills, abilities and needs, following the principles of supported employment. Assist in obtaining vocational, educational, and skills training opportunities for individuals through the use of a variety of available resources. Assist individuals in identifying available transportation options. As a specialist on the team, will be expected to develop generalist knowledge in the other disciplines represented on the team, as well as other general outreach, support, and treatment planning tasks in collaboration with the team. Qualifications High School Diploma or equivalent. Alternative education/experience in lieu of formal education will be considered. Valid Driver’s License, acceptable driving record, and reliable transportation. Reliability, willingness to learn, respect, and desire to see individuals thrive in stable educational, vocational, and/or employment opportunities. Experience working in vocational assessment/rehabilitation, job exploration, or similar case management field. Experience working with individuals with chronic and persistent mental illness and/or co-occurring mental health and substance use disorders.
