Case Coordinator I

New York, United States Center for Alternative Full time
Job DescriptionJob Description

Overview:

Manhattan Supervised Release provides supervision services to individuals arrested in Manhattan and placed on Pretrial Supervised Release. Supervised participants are mandated to comply with release reporting conditions in the community and may seek voluntary help with connection to behavioral health treatment and a variety of community-based supports and services.

CASES seeks a youth and young adult-focused Case Coordinator for its new youth initiative, Planning Alternatives & Creating Community (PACC) program under its Pretrial Services Supervised Release program. The youth-focused Case Coordinator will primarily work with justice system involved 16–27-year-old participants. Our team consists of Case Coordinators, Clinical Case Coordinators and Youth Engagement Specialists to support young people to achieve and maintain pretrial compliance while offering a range of youth focused services, such as incentivized work programs, to support early intervention and long-term success in the community. By utilizing the Rewire framework, we promote socio-emotional development while driving diversion from the criminal legal system, our team will promote proactive, sustained engagement with young people in the community as a catalyst toward pathways of education and employment, including by bridging to effective community partners eager to support our participants to achieve their goals. Working with the young-adult population requires flexibility, creativity, and persistence, and the Case Coordinator will be expected to engage with their participants (and individuals in their identified support networks) both at the office and in the community. The Case Coordinator will also be expected to work collaboratively with the rest of the team to generate and implement ideas for effective youth-focused engagement tactics (such as groups, field trips, and tangible incentives). This position is based in the Harlem office and requires some field work within the larger NYC community.

What you will be doing:

  • Provide supervision services to a caseload of predominantly 16–27-year-olds pretrial participants.
  • Conduct biopsychosocial intake assessments and initial orientation sessions with newly assigned participants to assess for needs, review supervision conditions, and create a reporting schedule.
  • Consistently assess and collaborate with a client centered approach to determine any goals and/or areas of concern that participants require or request support with.
  • Conduct individual supervision meetings and phone check- in's both at the office and in the community.
  • Work collaboratively with the team to generate and implement ideas for effective youth-focused engagement tactics (such as groups, field trips, and tangible incentives)
  • Effectively engaging and building a trusting rapport with participants and provide all services in a manner that is gender-responsive, trauma-informed, and that adheres to the core principles of risk-need-responsivity.
  • Utilize Motivational Interviewing (MI) and Cognitive Behavioral Therapy (CBT) techniques, such as Rewire to engage with participants and address patterns of behavior that have contributed to justice involvement (training will be provided)
  • Link participants to services and treatment based on their eligibility, preferences, and needs (as well as provide escort to community services as needed)
  • Maintain collaborative relationships with community providers and other identified individuals in participants’ support networks.
  • Work collaboratively with court staff and pretrial staff to facilitate and ensure assigned participant’s successful completion of mandated supervision appointments and court appearances.
  • Effectively schedule and maintain participant supervision appointments to meet necessary requirements for supervision and compliance.
  • Appropriately and punctually document all assessments, meetings, outreach efforts, and progress notes into Salesforce database to provide court stakeholders with accurate and up to date reporting on participant’s compliance.
  • Participate in individual and clinical group supervision to conference complex cases in order to provide best quality of care and services to participants.
  • Any other duties as required by the Borough Director and/or Team Leader for PACC.

What we are looking for:

  • Bachelor’s degree or equivalent experience
  • Minimum of 2-3 years’ experience working in human services, with young adult (16-27 yr. old) populations who experience substance use, mental health, complex trauma, and/or justice-system involvement.
  • Knowledge of community treatment, support services and resources tailored towards youth.
  • Knowledge of the criminal legal system, court processes and alternative to detention/incarceration services
