Job Title: Case Coordinator

Job Summary:

CASES is on the lookout for a Case Coordinator with exceptional organizational and attention to detail skills for our Intensive Services Unit. The Case Coordinator involves initiating Supervision Introduction/Orientation sessions with new entrants to outline necessary supervision conditions. The Case Coordinator will support participants in adhering to supervision schedules and legal proceedings, conduct regular assessments to identify service needs, and create collaborative service plans. The Case Coordinator emphasizes providing strength-based, gender-specific, and culturally sensitive services, guided by the principles of risk, need, and responsivity.

The aim is to reduce recidivism, foster community treatment engagement, and assist participants in achieving their personal goals for recovery and integration. The Case Coordinator should excel in teamwork, collaborate closely with a Peer Specialist, and be prepared for court duties and community fieldwork within NYC. Strong organizational and communication skills, along with efficient task prioritization, are essential. The initiative seeks to pioneer innovative methods to aid those in the Supervised Release Program facing significant challenges, ensuring their functional and programmatic success.

Salary: The salary for this role is $58,500 per year.

Shift Hours: This position is full-time, Monday through Friday from 1:00 pm to 9:00 pm.

Workplace Flexibility: Flexible (for roles that have days that can be partially remote, e.g., being able to draft notes at home after going to the office/community in the morning).

What you will be doing:

  • Deliver community-based direct services to a select group of pretrial participants, focusing on those with higher recidivism risks, mandated treatment requirements, and high-profile cases.
  • Quickly conduct Intake Assessments at the point of arraignment to identify and address immediate critical needs, ensuring continued program involvement and enrollment.
  • Engage in regular team meetings and case conferences to ensure seamless staff coordination, broad dissemination of participant information, and swift responses to urgent needs.
  • Review legal and court screening documents to facilitate admission and ongoing assessment of criminal history, psychosocial, developmental, and behavioral health needs, and supervision risks. This process helps identify engagement strategies, focus areas for behavioral change, and necessary supports to reduce re-arrest and promote community success post-program completion.
  • Act as the designated staff member for high-risk and high-needs cases, coordinating and participating in case reviews and conferences to ensure a collaborative approach for complex cases.
  • Facilitate effective collaboration with court service staff to ensure efficient enrollment of new participants into the program, while collecting essential information for ongoing service provision.
  • Connect participants to emergency, housing, education, vocational, employment, and treatment services based on their preferences and needs. Provide navigation to community services and foster positive relationships with community providers.
  • Use problem-solving and motivational interviewing techniques to engage and motivate participants in the change process. Ensure that supervision and case management approaches are culturally competent, gender-responsive, trauma-informed, and adhere to evidence-based risk-need-responsivity principles.

What we are looking for:

  • Bachelor’s degree in social work, Mental Health Counseling, Psychology, or a related field.
  • At least 1-2 years of experience in human services, working with individuals who have substance use disorders, co-occurring mental health and substance use disorders, and involvement in the Criminal Legal System.
  • Extensive knowledge of community treatment, support services, and resources.
  • Strong interpersonal skills and high emotional intelligence.
  • Ability to work effectively within a diverse team environment.
  • Must be able to sit for extended periods.
  • Must be able to navigate the five boroughs of NYC and perform field work in various weather conditions.

Additional, preferred skills we are looking for but are not required:

  • Preferred knowledge of the criminal legal system, court processes, and alternatives to detention/incarceration services.
  • Experience with databases, such as Salesforce, is preferred.
