The Coordinator is responsible for educating patients about their treatment plans and coordinating with Primary Care Providers and outpatient diagnostic testing facilities.
Key Responsibilities
- Collaborates closely with Primary Care Providers to enhance patient care plans through participation in daily meetings and monthly case reviews, facilitating appointments, and coordinating ongoing care needs.
- Engages with clinical staff to address challenges affecting patient care progress.
- Maintains and updates a dashboard with alerts and reminders to manage daily tasks, including visit status notifications, follow-up appointments, authorizations for medical reviews, and resolving billing discrepancies.
- Communicates with insurance companies for patient pre-certifications and consults with management regarding patient needs or escalated concerns.
- Provides front-desk support as necessary, documenting telephone interactions for appropriate routing to Providers.
- Acts as a liaison among Physicians, patients, and clinical staff to connect patients with essential internal and external resources.
- Offers suggestions for process improvements to enhance patient satisfaction and address concerns.
- Performs additional duties as assigned.
Qualifications
- Some college education; Bachelor's degree preferred.
- 3 years of experience in healthcare or customer service, or a related field.

- Engages with clinical staff to resolve issues that may impact patient care progression.
- Monitors and updates a dashboard with reminders and alerts to organize daily tasks, including visit status reminders, follow-up appointments, authorizations for medical reviews, and resolving billing errors.
- Contacts insurance carriers for patient pre-certifications and consults with management regarding patient needs or escalated issues.
- Provides front-end coverage as needed, documenting telephone encounters for routing to appropriate Providers.
- Acts as a liaison with Physicians, patients, and clinical staff to connect patients with relevant internal and external resources.
- Offers recommendations for process improvements to enhance patient satisfaction and address issues.
- Provides thoughtful and informative responses to patients while guiding them through their healthcare options.
- Maintains knowledge of medical terminology and practices through ongoing education.

Qualifications
4+ years of experience in healthcare or customer service, or a related field.
