Job Title: Youth Clinical Case Coordinator

Job Summary: 

CASES is looking for a Youth Clinical Case Coordinator. The Youth Clinical Case Coordinator will work with justice system involved 16–27-year-old participants to support their pretrial compliance and provide youth-focused services to promote early intervention and long-term success in the community. The coordinator will utilize the Rewire framework to promote socio-emotional development and drive diversion from the criminal legal system.

Salary: The salary for the Clinical Case Coordinator position is $65,000 annually.

Shift Hours: This position is full-time Monday through Friday from 9:00 am to 5:00 pm.

Location Address: 4 West 125th Street New York NY 10027.

Workplace Flexibility: Flexible (for roles that have days that can be partially remote, e.g., being able to draft notes at home after going to the office/community in the morning)

What you will be doing: 

  • Provide supervision services to a caseload of predominantly 16-27year old pretrial participants.
  • Conduct biopsychosocial intake assessments and initial orientation sessions with newly assigned participants to assess for needs, review supervision conditions, and create a reporting schedule.
  • Consistently assessing and collaborating with a client centered approach to determine any goals and/or areas of concern that participants require support around
  • Conduct individual supervision meetings both at the office and in the community.
  • Work collaboratively with the team to generate and implement ideas for effective youth-focused engagement tactics (such as groups, field trips, and tangible incentives).
  • Engaging and building a trusting rapport with high-risk participants who present with mental health symptoms, substance use concerns, and complex trauma histories.
  • Utilize Motivational Interviewing (MI) and Cognitive Behavioral Therapy (CBT) techniques, such as Rewire to engage with participants and address patterns of behavior that have contributed to justice involvement (training will be provided).
  • Link participants to services and treatment based on their eligibility, preferences, and needs (as well as provide escort to community services as needed).
  • Maintain collaborative relationships with community providers and other identified individuals in participants’ support networks.
  • Work collaboratively with court staff and pretrial staff to facilitate and ensure assigned participant’s successful completion of mandated supervision appointments and court appearances.
  • Effectively schedule and maintain participant supervision appointments to meet necessary requirements for supervision and compliance.
  • Appropriately and punctually document all assessments, meetings, outreach efforts, and progress notes into Salesforce database to provide court stakeholders with accurate and up to date reporting on participant’s compliance.

What we are looking for: 

  • Master’s degree in social work, Mental Health Counseling or equivalent professional clinical degree.
  • Minimum of 2-3 years' experience working in human services, with young adult (16-27 yr. old) populations who experience substance use, mental health, complex trauma, and/or justice-system involvement.
  • Extensive knowledge of community treatment, support services and resources tailored towards youth.
  • Knowledge of the criminal legal system, court processes and alternative to detention/incarceration services.

Preferred but not required:

  • Bilingual English/Spanish preferred
