Position Title: Clinical Case Coordinator
Position Summary:
The Center for Alternative is in search of a meticulous Clinical Case Coordinator for our Intensive Services Unit. In this capacity, the Clinical Case Coordinator will efficiently conduct orientation meetings with new participants to clarify the supervision conditions specified in their individualized plans.
The Clinical Case Coordinator will assist participants in maintaining their supervision appointments and court appearances throughout their legal processes. Regular assessments of participants' service requirements will be conducted, and collaborative plans will be developed to address these needs and objectives. By ensuring that services are strength-based, gender-responsive, and culturally attuned, the Clinical Case Coordinator will adhere to essential principles of risk, need, and responsivity to mitigate recidivism, enhance community treatment engagement, and support participants in achieving their personal treatment, recovery, and reintegration goals.
This initiative aims to implement innovative strategies for working with individuals enrolled in the Supervised Release Program who face significant challenges affecting their daily functioning and overall program participation. The focus group of program participants grapples with issues related to mental health, substance use, employment, and medical health. The team promotes proactive and sustained community engagement as a catalyst for individual transformation by connecting participants with community partners and resources dedicated to supporting their potential.
Key Responsibilities:
- Provide dedicated support to a specialized group of pretrial participants, particularly those with elevated risks of reoffending, mandated treatment requirements, or involvement in high-profile cases.
- Quickly evaluate participants' immediate critical needs at the point of arraignment to ensure ongoing program involvement and enrollment.
- Review legal and court screening documents to facilitate admission and conduct continuous assessments of criminal history, psychosocial, developmental, and behavioral health needs. Identify engagement strategies, focus on behavioral change, and necessary supports to minimize re-arrest and promote successful community integration.
- Collaborate with court services staff to efficiently enroll new participants, gathering essential information for continued service provision.
- Connect participants with emergency, housing, education, vocational, employment, and treatment services based on their needs and preferences. Provide navigation assistance to community services and foster positive relationships with community providers.
- Engage in clinical supervision, case reviews, staff meetings, and in-service training to enhance skills in clinical practice, motivational interviewing, trauma-informed care, supervision, case management, and self-care.
Qualifications:
- Master's degree in social work, Mental Health Counseling, Psychology, or a related clinical discipline.
- One to two years of experience delivering clinical services to individuals, families, or groups facing challenges in Behavioral Health and the Criminal Legal System.
- Strong interpersonal skills and emotional intelligence.
- Ability to work effectively within a diverse team environment.
Preferred Skills:
- Experience providing direct clinical services in a field-based setting is strongly preferred.
- Active licensure in a clinical discipline in New York State is preferred.
