Position Title: Clinical Case Coordinator
Position Summary:
The Center for Alternative is seeking a detail-oriented Clinical Case Coordinator for our Intensive Services Unit. In this capacity, the Clinical Case Coordinator will promptly conduct introductory meetings with new participants to clarify the supervision conditions outlined in their plans.
The Clinical Case Coordinator will assist participants in maintaining adherence to supervision appointments and court appearances throughout their legal processes. Regular assessments of participants' service requirements will be conducted, leading to the development of collaborative plans that address these needs and objectives. By ensuring that services are strength-based, gender-responsive, and culturally appropriate, the Clinical Case Coordinator will adhere to core principles of risk, need, and responsivity to mitigate recidivism, enhance community treatment engagement, and support participants in achieving their personal treatment, recovery, and reintegration goals.
This initiative will implement innovative strategies for working with individuals in the Supervised Release Program who face extensive needs and challenges impacting their daily functioning and overall program engagement. The target group of program participants deals with issues related to mental health, substance use, employment, and medical health. The team fosters proactive, sustained community engagement as a catalyst for individual transformation by connecting participants with community partners and resources eager to assist them in realizing their full potential.
Salary Range: $62,500 - $67,500 per year.
Shift Hours: This position is full-time, operating Monday through Friday from 1 PM to 9 PM.
Workplace Flexibility: Flexible
Key Responsibilities:
- Provide dedicated support to a specialized group of pretrial participants, particularly those with higher risks of reoffending, mandated treatment needs, or involvement in high-profile cases.
- Quickly evaluate participants' immediate critical needs at the point of arraignment to ensure ongoing program involvement and enrollment.
- Review legal and court screening documents to facilitate admission and conduct ongoing assessments of criminal history, psychosocial, developmental, and behavioral health needs. Identify engagement strategies, behavioral change focus, and necessary supports to minimize re-arrest and promote successful community integration.
- Collaborate with court services staff to efficiently enroll new participants, gathering essential information for continued service provision.
- Connect participants with emergency, housing, education, vocational, employment, and treatment services based on their needs and preferences. Provide navigation assistance to community services and foster positive relationships with community providers.
- Engage in clinical supervision, case reviews, staff meetings, and in-service training to enhance skills in clinical practice, motivational interviewing, trauma-informed care, supervision, case management, and self-care.
Qualifications:
- Master's degree in social work, Mental Health Counseling, Psychology, or another relevant clinical discipline.
- One to two years of experience providing clinical services to individuals, families, or groups facing challenges in Behavioral Health and the Criminal Legal System.
- Strong interpersonal skills and emotional intelligence.
- Ability to work effectively within a diverse team environment.
Preferred Skills:
- Experience providing direct clinical services in a field-based setting is strongly preferred.
- Active licensure in a clinical discipline in New York State is preferred.
