Job Overview

At Sea Mar Community Health Centers, we are dedicated to providing high-quality healthcare services to diverse communities. We are seeking a skilled Social Services Specialist to join our Maternity Support Services Program located at our Salmon Creek Medical Clinic.

The successful candidate will conduct initial screenings and comprehensive assessments, develop intervention plans, provide brief treatment, and coordinate transition services for long-term counseling and Infant Case Management. The ideal candidate will have excellent communication skills, knowledge of evidence-based screening tools, and experience working with underserved populations.

Key Responsibilities:

  • Conduct initial screenings and comprehensive assessments of patients' needs.
  • Develop intervention plans based on assessment results.
  • Provide brief treatment and coordinate transition services for long-term counseling and Infant Case Management.
  • Consult with the interdisciplinary Maternity Support Services team to ensure patient needs are met.

Requirements:

  • Bachelor's degree from an accredited university.
  • Current Certified Counselor or Chemical Dependency Counselor certification through the State of Washington Department of Health.
  • Experience working with underserved populations, including those with limited English speaking skills, homelessness, and migrant workers.
  • Knowledge of medical terminology and electronic health record platforms.
