The Medical Social Worker position at PeaceHealth Southwest Medical Center offers a unique opportunity to work with patients and families navigating complex healthcare transitions. As a key member of our Care Management team, you will play a vital role in ensuring seamless transitions from acute care to post-acute settings.

Your primary responsibilities will include:

  • Assessing patients' needs and developing comprehensive discharge plans in collaboration with interdisciplinary teams
  • Counseling patients and families to facilitate successful transitions and improve quality of life
  • Coordinating with community resources to ensure access to necessary services and support
Required Skills and Qualifications

To be successful in this role, you will need:

  • A Master's degree in social work (MSW) or a related field
  • A Washington State credential, such as an Agency Affiliated Counselor Registration (CAAR), to practice as a medical social worker
  • At least two years of experience in a healthcare setting or community agency dealing with health and/or welfare issues
  • Excellent communication and problem-solving skills, with the ability to work effectively in a fast-paced environment
