About the Job

Peace Health is seeking a skilled Care Management Specialist to join their team in Vancouver, Washington. This part-time position will work in the Care Management department at Southwest Medical Center.

Job Summary

This role involves identifying and interacting with medically and psychosocially complex patients and families who require care management services. The successful candidate will be responsible for coordinating discharge planning services, working collaboratively with the RN Care Management team and other healthcare professionals.

Key Responsibilities

  • Screen and identify patients who require care management services based on high-risk criteria.
  • Assess, develop, implement, and monitor comprehensive discharge plans of care through an interdisciplinary team process.
  • Collaborate with the care team to identify problems or needs that require special planning, intervention, teaching, or follow-up.
  • Coordinate and facilitate improved ability to comply with treatment plans, counseling or support needed to cope with situations, and improved access to appropriate levels of care due to lack of financial resources or available services.
  • Actively support measures that promote effective use of resources.

Qualifications

The ideal candidate will possess a Master's degree in Social Work (MSW) and a Washington State credential to practice as a social worker. A minimum of two years' experience in a healthcare setting or community agency dealing with health and/or welfare issues is preferred. Demonstrated knowledge of community health, welfare, and social agencies is essential.

Compensation and Benefits

The estimated annual salary for this position is $56,190 - $82,170, depending on professional experience, education, and qualifications.
