Sea Mar Community Health Centers is a leading provider of comprehensive health and human services in Washington State. We are seeking an experienced Pregnancy Care Navigator to join our team at the Salmon Creek Medical Clinic.

About the Role:

The Pregnancy Care Navigator will be responsible for providing support and guidance to expectant mothers, ensuring they receive high-quality care throughout their pregnancy journey. This role involves collaborating with healthcare providers, community resources, and social services to address the unique needs of each patient.

Responsibilities:

  • Conduct prenatal intake and gather past medical history
  • Document patient information in electronic health records
  • Provide updates and information regarding shared OB and MSS clients
  • Assist patients with managing their pregnancy and linking them to social support services
  • Perform referrals, scheduling, phone calls, and assist MSS groups as assigned
  • Participate in weekly interdepartmental meetings
  • Manage referral process, including tracking and ensuring patient attendance
  • Complete prenatal contracts and paperwork with clients
  • Send letters and perform follow-up phone calls for planned visits
  • Advocate for patient services with community, social service, and medical providers
  • Connect patients to Sea Mar and non-Sea Mar resources, tracking available resources
  • Track patient adherence to plan of care and communicate outcomes to providers
  • Prepare monthly reports for provider meetings and update prenatal panel reports
  • Ensure OB Database is updated regularly

Qualifications:

  • High school diploma and continued education in health or social services, or at least one year of experience in those areas
  • 2 years' experience in a health and human services setting preferred
  • BLS CPR certification required within 90 days of hire date and maintained throughout employment
