Job Summary

We are committed to delivering exceptional patient care and are seeking a highly motivated Medical Social Worker - Hospice to join our team. As a Per Diem/Relief position, you will have the opportunity to work in various healthcare settings.

Duties and Responsibilities
  1. Perform holistic evaluations of patients and develop comprehensive plans of care
  2. Coordinate and modify care plans as necessary, collaborating with patients, families, and healthcare teams
  3. Provide expert social work services to patients and educate caregivers on hospice care and support
Qualifications
  • Masters degree in Social Work from a recognized institution
  • Washington state licensed Social Worker
  • Basic Life Support certification
