Job Summary

PeaceHealth seeks a skilled Medical Social Worker to provide direct social work services in Home Health and/or Hospice settings. This Per Diem/Relief position involves utilizing the professional healthcare process of assessment, problem identification/planning, action/intervention, and clinical goal/outcome evaluation.

Key Responsibilities:
+ Providing holistic evaluations of patient/home environments, developing appropriate plans of care, initiating discharge planning, evaluating progress towards goals, and monitoring achievement of desired outcomes.

+ Coordinating social work services with inter/multi-disciplinary teams to ensure smooth patient care flows throughout the continuum.

+ Evaluating patient/social needs, making referrals to other services, agencies, and community resources as clinically indicated in collaboration with case managers and primary physicians.

Requirements:
+ Master's Degree in Social Work from an accredited school.

+ Advanced License in Social Work (e.g., LCSW) or Associate Advanced License (e.g., LSW).

+ Basic Life Support certification.
